Proof of Citizenship (Any 2) :-
- birth certificate
- Voter Identity Card / Aadhaar Card
- Passport / Domicile Certificate of parents
- PAN card
- Driver's license
- Insurance policy
- School Leaving Certificate
- Domicile Certificate (if born outside India)
Residential Proof (Any 2) :-
- Ration sheet
- Letter from Housing Society / Maintenance Receipt
- Rent Agreement / Receipt / Real Estate Registration Documents
- Government allotment letter
- 1st and 12th Month Telephone / Electricity Bill / Water Bill / Metropolitan Gas Bill
- Bank Passbook First Page Shadow Copy / Income Tax Certificate
Name Change Issues (Any 1) :-
- Marriage Certificate / Affidavit / Divorce Document / Agreement
- Self-declared name proof or two valid proofs of change of names in Election Commission electoral roll / as required.
- Two evidences of a new change in name
